Terms of Service

1. Introduction and Acceptance
LeadForms is operated by Oso Grajales, a sole proprietor based in Phoenix, Arizona. By creating an account or using the LeadForms service, you agree to these terms.
If you do not agree to any portion of these terms, do not use the service. Continued use of LeadForms after these terms are updated constitutes acceptance of the changes.
2. Description of Service
LeadForms is a scored lead capture platform built for local service businesses. The service includes multi-step lead capture forms, AI-powered lead scoring, SMS and email notifications, booking calendar integration, and a lead management dashboard.
LeadForms is a software-as-a-service platform. We do not guarantee any specific business results. The quality of leads generated through your forms depends on many factors outside our control.
3. Account Registration
- You must provide accurate information when creating an account.
- You are responsible for maintaining the security of your account credentials.
- You must be 18 years or older to use the service.
- One account per person. You may not share accounts with other individuals.
4. SMS Notification Terms
Business owners who add a phone number to their notification settings consent to receive transactional SMS lead alerts from LeadForms.
By saving a phone number in notification settings, you confirm that you are the owner or authorized user of that phone number and that you consent to receive SMS alerts from LeadForms at that number.
Message and data rates may apply. Standard carrier rates from your mobile provider apply to any SMS you receive.
Message frequency depends on lead volume. Typically 1 to 10 messages per day.
To stop receiving SMS, reply STOP to any message or remove your phone number from notification settings.

LeadForms sends transactional alerts only. No marketing SMS will be sent.
5. Lead Capture Forms and End User Data
When your LeadForms form is filled out by a prospect, that prospect's data is your responsibility. You must have a lawful basis for collecting prospect data via your form.
- You must not use LeadForms to collect data in violation of applicable laws, including GDPR, CCPA, or TCPA.
- You are responsible for your own privacy policy and terms presented to your form respondents.
- LeadForms processes prospect data on your behalf as a data processor.
6. Acceptable Use
- You may not use LeadForms for illegal purposes.
- You may not use LeadForms to collect data from minors under 13.
- You may not spam, abuse, or harass individuals through any LeadForms feature.
- You may not attempt to reverse engineer, copy, or resell the LeadForms platform without written permission.
Violation of acceptable use may result in immediate account termination without refund.
7. Payment and Billing
- Paid plans are billed monthly via Stripe.
- All fees are in USD.
- Subscriptions auto-renew unless canceled.
- You may cancel at any time. Cancellation takes effect at the end of the current billing period. No refunds are issued for partial months.
- LeadForms reserves the right to change pricing with 30 days notice.
8. Free Plan Limitations
The free plan includes unlimited lead submissions, 1 AI form generation per day, and 2 logic rules per form. LeadForms reserves the right to modify free plan limits at any time.
9. Intellectual Property
- LeadForms and all related trademarks, logos, and product names are the property of Oso Grajales.
- Your data remains your property.
- By using the service, you grant LeadForms a limited license to process your data to provide the service.
10. Disclaimer of Warranties
The service is provided as-is without warranty of any kind. LeadForms does not guarantee uptime, accuracy of AI scoring, or specific business outcomes. Use of the service is at your own risk.
11. Limitation of Liability
LeadForms liability is limited to the amount you paid in the 12 months prior to any claim. LeadForms is not liable for indirect, incidental, or consequential damages.
12. Termination
- Either party may terminate at any time.
- LeadForms may terminate accounts for violation of these terms.
- Upon termination, your data will be retained for 30 days, then deleted, unless you request earlier deletion.
13. Governing Law
These terms are governed by the laws of the State of Arizona. Any disputes will be resolved in Maricopa County, Arizona.
14. Changes to Terms
We may update these terms at any time. Material changes will be communicated via email. Continued use of the service after changes constitutes acceptance.
