1. Introduction
LeadForms is operated by Oso Grajales, a sole proprietor based in Phoenix, Arizona. This policy describes how we collect, use, and protect the personal data of people who use leadcontactforms.com and all related services.
This policy applies to anyone who creates a LeadForms account, anyone whose data is submitted through a LeadForms-powered form, and anyone who interacts with the LeadForms platform in any way.
2. Information We Collect
LeadForms collects information in three categories.
- Information you provide. When you create an account or submit a lead capture form, we collect your name, email address, and phone number. We also collect any answers you provide to questions on a LeadForms-powered form.
- Information collected automatically. When you visit the LeadForms platform we collect your IP address, browser type, device type, the pages you visit, and the time you spend on each page. We use this information to operate the service and understand product usage.
- Information from third parties. We receive payment information from Stripe and authentication information from Supabase. We do not directly handle credit card numbers.
- SMS notification data. Business owners who use LeadForms can enter a phone number in their dashboard notification settings to receive SMS lead alerts. We store that phone number and transmit it to our SMS provider for delivery.
3. How We Use Your Information
- To operate and improve the LeadForms platform.
- To send transactional SMS alerts to business owners who have opted in via their dashboard notification settings.
- To process payments via Stripe.
- To send transactional email notifications about lead activity.
- To analyze usage patterns and improve the product over time.
We do not sell your information to third parties. Ever. We do not use your information for advertising purposes.
4. SMS Communications
Business owners who configure a phone number in their LeadForms dashboard notification settings consent to receive SMS alerts when new leads are captured.
Message frequency varies based on lead volume. Typically 1 to 10 messages per day.
Message and data rates may apply. Standard carrier rates from your mobile provider apply to any SMS you receive.
To opt out, reply STOP to any message or remove your phone number from notification settings in your dashboard. Opting out stops all future SMS alerts.

LeadForms will never send marketing SMS. All SMS is transactional lead alert content only.
5. Data Sharing
We share data only with the third-party processors required to deliver the service.
- Stripe. Payment processing. Subject to Stripe's privacy policy.
- Supabase. Database and authentication infrastructure. Data stored in United States data centers.
- Twilio. SMS delivery infrastructure. Message content and recipient phone numbers are transmitted to Twilio for delivery.
- Resend. Transactional email delivery.
We do not share data with any other third parties.
6. Data Retention
Lead data is retained as long as your account is active. You may delete your account and all associated data at any time by contacting support@leadcontactforms.com.
Stripe retains payment records per their own retention policies, which we cannot override.
7. Security
- All data is encrypted in transit via TLS.
- Database access is protected by row-level security policies.
- Payment data is handled entirely by Stripe and never stored on LeadForms servers.
8. Your Rights
- You may request a copy of your data at any time.
- You may request deletion of your data at any time.
- You may opt out of SMS at any time by replying STOP or removing your phone number from dashboard settings.

9. Changes to This Policy
We will notify users of material changes via email. Continued use of the service after changes constitutes acceptance of the updated policy.
